Your feedback is an important part of our ongoing effort to increase the effectiveness of our program. We appreciate you taking the time to help us better serve your needs as well as the needs of your clients. Are you supervising the work of a TVP Volunteer?In an effort to follow-up with our clients' progress, we ask that you please complete this evaluation form * after the first three months of the volunteer's assignment. We encourage you to set aside time to share your feedback with your client. Your evaluation will be shared only with the volunteer and his/her referring counselor.
